Hours:
     – Mondays through Thursdays: 9 am to 4 pm.
     – Fridays: 10 am to 12 pm (Virtual visits)
     – Saturdays, Sundays, & holidays: Virtual visits may be available Contact us for availability. 

After hours, please leave a voice message or send a text and we will reply within TWO business days.

Scheduling Appointments
You may schedule an appointment in one of the following ways.
(1) Enter a request in the secure Patient Portal
(2) Call the office during office hours or leave a voice message with your contact information and we will call you back within one business day

If you need to change an appointment that has already been scheduled, please notify us 48 hours before the scheduled appointment to allow us to offer that time to another patient. You may do so by one of the following methods:
(1) Send us a request via the secure Patient Portal
(2) Send us a request via text message 
(3) Call the office during business hours or leave a voice message with your contact information and we will call you back within one business day

Secure Patient Portal
Please use the Patient Portal for communication with the practice. You can post questions, request information, or appointments. You will be able to receive copies of labs, imaging studies, instructions from doctor or staff via the portal.

Your First Appointment
Before your first appointment, please fill out the forms on the secure Patient Portal. You may also down load them from the Forms Tab, fill them out, and bring them to the appointment along with your insurance card. 

Please use the Guide to Medical Office Visit to help make the best of your office visit. 

Payment is expected at the time of your office visit. If your insurance plan requires you to pay a co-payment, it is due at the time of the visit.

If you have special circumstances that affect your ability to pay or meet our payment policy, we would like to discuss them with you and will make every effort to accommodate them. If you are unable to pay for services at the time they are performed, we may be able to set up a payment plan. If you think you will need a payment plan, please let us know in advance so that we can better assist you. If you have any questions regarding this policy, please talk to the office assistant or manager.
