– Mondays through Thursdays: 9 am to 4 pm.
– Fridays: 10 am to 12 pm (Virtual visits)
– Saturdays, Sundays, & holidays: Virtual visits may be available Contact us for availability.
Office closed during vacations.

Scheduling Appointments
You may schedule an appointment
Call or Text the office and share your contact information and we will call you back within 1-2 business days
If you need to change an appointment that has already been scheduled, please notify us 48 hours before the scheduled appointment to allow us to offer that time to another patient. You may do so by one of the following met
(1) Send us a request via text message
(2) Call the office during business hours or leave a voice message with your contact information and we will call you back within 1-2 business days
Your First Appointment
Before your first appointment, please fill out the forms needed to have an appointment. You may down load them from the Forms Tab, fill them out, and bring them to the appointment along with your insurance card.
Please use the Guide to Medical Office Visit to help make the best of your office visit.
Payment is expected at the time of your office visit. If your insurance plan requires you to pay a co-payment, it is due at the time of the visit.
If you have special circumstances that affect your ability to pay or meet our payment policy, we would like to discuss them with you and will make every effort to accommodate them. If you are unable to pay for services at the time they are performed, we may be able to set up a payment plan. If you think you will need a payment plan, please let us know in advance so that we can better assist you. If you have any questions regarding this policy, please talk to the office assistant or manager.
